COVID Waiver
     

    Owatonna Curling Club (“OCC”) is pleased to announce the opening of curling season.  Curling activities are permitted to resume subject to guidance set forth by the Center for Disease Control and Prevention (“CDC”), Minnesota Department of Health (“MDH”), any federal, state, or local governmental orders, and any additional protocols instituted by the OCC Board of Directors to best protect OCC members, employees, staff, and visitors from the risks associated with COVID-19. The known risk factors associated with COVID-19 are limited.  The CDC believes that people of all ages are at risk of experiencing severe symptoms from COVID-19, though older adults and people with serious underlying health conditions may be at a higher risk. While OCC has taken drastic and unprecedented measures to reduce the risk of COVID-19, it is unfortunately impossible to guarantee our members, employees, staff, and visitors full protection from all risks of COVID-19 exposure.

    Further, attending OCC and/or participating in the sport of curling at OCC could increase your risk of contracting COVID-19.  With this knowledge, you agree that your attendance at OCC and participation in curling is voluntary and you agree to abide by directives designed to lower the risk of COVID-19 exposure including, but not limited to, wearing masks, social distancing (6 feet apart), washing hands, and non-sharing of equipment.

    In accordance with the foregoing and as a consideration of membership at OCC, the undersigned acknowledges and agrees as follows:

    1. Curling at OCC includes possible exposure to and illness from infectious diseases including, but not limited to, COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.

    2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of OCC, its employees, staff, or others, and assume full responsibility for my attendance and participation in the sport of curling at OCC.

    3. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless OCC, its officers, officials, agents, employees, and/or other participants, sponsoring agencies, sponsors, and advertisers, with respect to any illness, disability, death, loss or damage to person or property whether arising from the negligence of OCC or otherwise, to the fullest extent permitted by law.

    I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. 



Injury Waiver

    In consideration of being allowed to participate in any way in the programs of the Owatonna Curling Club and the
    UNITED STATES CURLING ASSOCIATION, INC. (“USCA”), their related events and activities, the undersigned 
    acknowledges, appreciates and agrees that: 

    1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and
    2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, or others, and assume full responsibility for my participation; and
    3. I will agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual, significant hazard during my presence and participation, I will remove myself from participation and bring such to the attention of the Owatonna Curling Club and the USCA immediately; and
    4. NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE THE OWATONNA CURLING CLUB AND THE USCA, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used for this activity (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE

Membership Selection Instructions

To register for the current season:

  1. To register for ALL Owatonna Curling Club leagues,
    • First select OCC Membership by checking the round check-box.

    • Next, select the square check-box for each league that you wish to curl in.  Fill in the requested information. 

    • Each curler must register separately.  Listing your teammates does not register them for the season.  

    •  Enter all known team member names for leagues where you can choose your own teams, though please note that each team member must submit their own form. Not a full team? Please write TBD in the empty positions. If you will have more than four members on the team, please write in additional players in the "Notes for Convenor" section.

  2. Next select from the Additional Registration Options, i.e. team did not help with setup, etc.

  3. Then, select discounts, if applicable, from the Registration Discount Options list.

  4. Confirm that you have entered your information correctly by selecting  the Registration Accuracy box.  Read the payment instructions there.

  5. Select  Validate Registration and Calculate Fees at the bottom to proceed to the next step.  If you get an error message, make corrections and select Validate Registration and Calculate Fees again.

  6. Review registration and fee calculation on the next page.  If correct, then select "Complete Registration" at the bottom of the page.

**IMPORTANT** In order to be fully registered, you must click "Valid Registration and Calculate Fee" at the bottom of this page AND click on "Complete Registration" on the following page. Without clicking on "Complete Registration", you will not be registered.

Registration Accuracy Confirmation

    Paying your dues:

    Dues payment is requested no later than your first day of league play. 

    Payment can be made by Paypal or Check.

      • NSF checks will be charged all fees necessary to cover OCC bank fees.  Membership will be inactivated until the matter is resolved.
